Welcome to Infinite Healing’s forms and resources page. If you have booked your initial visit with one of our practitioners, please complete and print the appropriate health-history form and bring it with you to your first appointment.
Some documents can be completed digitally and emailed or faxed to the clinic when finished. If you send documents electronically, always confirm you are using the correct email address or fax line and verify that the clinic has received them successfully.
